  1. Note: Churches were categorized according to whether they met a-priori criteria for implementation at 12 and 24 months. A score of 3 (“about monthly” or “some of the time,” depending on the item) or 4 (“about weekly” or “almost all of the time,” depending on the item) was defined as acceptable implementation. These data were limited to the 70 churches where FAN Coordinators completed the 24-month survey
